How Tirzepatide works

Tirzepatide binds both the GIP and GLP-1 receptors. GLP-1 activity slows gastric emptying and amplifies glucose-dependent insulin release, while the GIP arm improves insulin sensitivity and how adipose tissue handles incoming lipids. Two receptors working together produce a stronger satiety and glycemic signal than a GLP-1 agonist alone at comparable exposure.

SURMOUNT-1 (72 weeks, obesity, n≈2,539)

Mean body-weight reduction of about 15% at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg versus 3.1% on placebo; roughly a third of participants on the highest dose lost ≥25% of body weight.

SURPASS programme (type 2 diabetes)

HbA1c reductions of approximately 1.9–2.6 percentage points across doses, outperforming semaglutide 1 mg, degludec and glargine comparators, with most participants reaching HbA1c below 7%.
